In recent years, there have been concerted efforts to better recruit, support, and retain diverse faculty, staff, and trainees in academic medicine. However, many institutions lack comprehensive and strategic plans to provide support to retain and recruit individuals from historically underrepresented groups. In this article, we itemize specific mechanisms through which institutions can support diverse individuals with the goal of improving inclusion and belonging in the workforce to better reflect the diversity of the intended patient and research participant population.
Predictors of quality of life ratings for persons with dementia simultaneously reported by patients and their caregivers: the Cache County (Utah) Study

Background: Quality of life (QOL) is frequently assessed in persons with dementia (PWD) through self- and/or proxy-report. Determinants of QOL ratings are multidimensional and may differ between patients and caregiver proxies. This study compared self- and proxy-reported QOL ratings in a population-based study of PWD and their caregivers, and examined the extent to which discrepancies in reports were associated with characteristics of the PWD.
Methods: The sample consisted of 246 patient/caregiver dyads from the initial visit of the Cache County Dementia Progression Study, with both members of the dyad rating PWD QOL. PWD age, gender, cognitive impairment (Mini-Mental State Examination), neuropsychiatric symptoms (Neuropsychiatric Inventory; NPI), dementia severity (Clinical Dementia Rating), medical comorbidities (General Medical Health Rating), and functional impairment (Dementia Severity Rating Scale) were examined as correlates of self- and proxy-reported QOL ratings and the differences between the QOL reports.
Results: Self- and proxy-reported PWD QOL ratings were only modestly correlated. Medical comorbidity was associated with self-report whereas NPI was associated with proxy-report. Dementia severity was associated with discrepancies in self- and proxy-report, with worse patient cognition associated with poorer proxy-reported QOL ratings.
Conclusions: PWD self- and proxy-reported QOL ratings are associated with different variables. Discrepancies between PWD and caregiver perceptions of PWD QOL should be recognized, particularly in cases of more severe dementia.

Summary
While malignancies in women of reproductive age are relatively uncommon, they are, after accidents, the most common cause of death in women aged 20-39 in the USA. Malignant melanoma was the most frequently diagnosed cancer occurring in pregnancy, followed by cervical cancer and breast cancer, reflecting the high melanoma risk in the fair-skinned Swedish population. The incidence of melanoma increases steadily from the age of 20 years and thus is a relatively common cancer in women during their reproductive years and is more common than breast cancer in the under-30s. Thyroid cancer is 2.5 times more common in women than men and the incidence varies between different populations owing to genetic and environmental factors. Gestation trophoblastic neoplasia is a malignancy intimately related to pregnancy. Improved data could be obtained by the central population-based registries formally collecting data on cancer during pregnancy.
Psychological factors in people at ultra-high risk of psychosis: comparisons with non-patients and associations with symptoms

Background. There have been recent advances in the ability to identify people at high risk of developing psychosis. This has led to interest in the possibility of preventing the development of psychosis and provides the opportunity to investigate psychological mechanisms that may confer vulnerability to psychosis.
Method. Fifty-eight patients at ultra-high risk of developing a first episode of psychosis were compared with 56 non-patients matched for age and occupational status on measures of meta-cognition, schizotypal traits, dysfunctional attitudes and distress.
Results. Analyses of covariance revealed that people at high risk of developing psychosis scored higher on measures of cognitive vulnerability, including negative meta-cognitive beliefs, beliefs about rejection and criticism from others, and discrepancies in self-perception, schizotypal traits and general mental distress. Correlational analyses revealed that negative meta-cognitive beliefs, dysfunctional attitudes and beliefs about rejection and criticism from others were positively associated with several dimensions of symptomatology in at-risk mental states (ARMS) patients.
Conclusions. Cognitive and personality factors appear to characterize people at high-risk of developing psychosis and are associated with their distressing experiences. The clinical implications of these findings are discussed.

Background
Advances in the ability to identify people at high risk of developing psychosis have generated interest in the possibility of preventing psychosis.
AimsTo evaluate the efficacy of cognitive therapy for the prevention of transition to psychosis.
MethodA randomised controlled trial compared cognitive therapy with treatment as usual in 58 patients at ultra-high risk of developing a first episode of psychosis. Therapy was provided over 6 months, and all patients were monitored on a monthly basis for 12 months.
ResultsLogistic regression demonstrated that cognitive therapy significantly reduced the likelihood of making progression to psychosis as defined on the Positive and Negative Syndrome Scale over 12 months. In addition, it significantly reduced the likelihood of being prescribed antipsychotic medication and of meeting criteria for a DSM – IV diagnosis of a psychotic disorder. Analysis of covariance showed that the intervention also significantly improved positive symptoms of psychosis in this population over the 12-month period.
ConclusionsCognitive therapy appears to be an acceptable and efficacious intervention for people at high risk of developing psychosis.

Fast relaxation of stresses lower than the yield stress is demonstrated in Bombyx mori(silkworm) cocoon silk and Nephila clavipes (spider) major ampullate silk (MAS; dragline). Stress relaxation and creep make natural silk unsuitable as a long-term load-bearing material. Instead, silk-like materials are better suited to applications in which energy dissipation is important, and in which high loads need to be withstood on a once-off basis for only very short periods of time. Examples might include use as a ballistic material that arrests the penetration of fragments from the explosion of a pressure vessel, an aircraft luggage container, or a tyre. Treatment in a domestic microwave oven is shown to significantly reduce the rate of stress relaxation in both silkworm cocoon and spider MAS. Except for ductility, the tensile properties of cocoon silk measured in constant strain rate experiments are enhanced by this treatment. Initial experiments on MAS suggest that the tensile properties of this material also are enhanced by exposure to microwaves, in this case with the exception of initial modulus.
